THE BEST CHRISTMAS PAGEANT EVER: Parent Permission Form
(This form is to be completed by the parent/guardian.)

In an effort to ensure that both students and their parent/guardian(s) are fully aware of the commitment being made by students who participate in Dixon-Smith Middle School Theater productions, we ask that the parents/guardian review the content of this form with their student initialing and signing each section.,

This form must be submitted by October 6  at 5pm in order for a student to be considered for casting.

REGULAR REHEARSALS:
Rehearsals begin for the full cast on Tuesday, October 11. After-School-6:00
Rehearsal:  Monday-Friday After-School until 6:00
A Calendar with more specific dates will be handed out after the play is cast. As for now, plan on rehearsal Tuesday-Friday

PERFORMANCES:
December 1-2 at 7:00 pm

As the parent/guardian, I understand that if my student is cast or joins the crew, it will include committing to the schedule detailed above. *

I understand that we, the student and parent/guardian, can submit conflicts with the above schedule on this form, and that they will be taken into consideration in the casting and crew staffing of the play. Also, conflicts added after the play is cast/staffed may result in the student being removed from their role in the production. Should unexpected conflicts arise, we will notify the director as soon as possible so that they can be taken into consideration. *
Conflicts with the Schedule:
This production may include costumes, minor set pieces, props and portable equipment being provided to students in order to execute the production, and that these items might be in our home. Students may also be asked to put together / purchase costume items of their own. *
