**Important Notice: Authorization Form for Euthanasia Service**

- **EUTHANASIA AND/OR CREMATION IS AN IRREVERSIBLE AND FINAL PROCESS.**
**Ownership Confirmation:**
- I/We represent that I/We have the right to authorize the euthanasia of the Pet and warrant that I/we are the Owner or an Agent of the Owner.

**Rabies Exposure:**
- To the best of my knowledge, my pet has not bitten, scratched, or otherwise potentially exposed any person or others to rabies in the past ten (10) days. I understand that if the animal described above has bitten or otherwise potentially exposed any person within the time specified, a rabies test must be performed.

**Euthanasia Understanding:**
- I understand that euthanasia is the act of ending the life of an animal in a painless way to prevent any unnecessary suffering. The information I have provided is accurate and complete. I understand that my wishes may be carried out immediately upon my signing this agreement.

**Transportation Details:**
- I/We acknowledge that during the transportation of my pet from the pick-up location to the pet funeral services and crematory, Angel Paws USA reserves the right to determine the type of vehicle used, which may include cars, vans, SUVs, or pickups. The choice of vehicle will be based on logistical considerations, including the size and weight of the pet. The owner has no control over the type of vehicle used.
- I/We also acknowledge that Angel Paws USA may transport more than one pet at a time for efficiency. We understand that all pets will be handled with care and respect during transportation.
- Angel Paws USA will always strive to arrive at the pick-up location within 1-3 hours, providing prompt and compassionate service to pet owners during their time of need.

**Variable Response Time:**
- I/We understand that response times for removal may vary depending on the amount of calls and other factors. While we aim for prompt service, there may be instances where response times exceed 1-3 hours. In such cases, no refund will be issued if the response time is within 12 hours.

**Choice of Crematory:**
- I/We acknowledge that Angel Paws USA acts as a pet funeral service and reserves the right to choose the crematory for the pet. All selected crematories will undergo extensive evaluation to ensure they meet the minimum standards of care for Angel Paws USA. We are committed to selecting the best crematory in the area to provide exceptional service.

**Payment Agreement & Terms Disclosure:**

**Payment Process:**
- A non-refundable deposit of $350 (Three Hundred Fifty Dollars) for Euthanasia Service is due upon completion of this Form. This deposit is not refundable under any circumstances. The remaining balance will be automatically charged, including any additional items you select, once the removal has been completed.
- If the final payment cannot be processed, a $50 fee will apply.

If no License Vet Available In The Area Additional Fee For Travel/Time Will Apply. Customer Will Be Advise At Time Of Booking.

**Timely Collection:**
- If cremains and/or the Package are not prepared for collection within the estimated 7-10 days after notification, a storage fee of $50.00 will apply. If not collected within 15 days of notification of readiness, they will be disposed of, and no refund will be issued.

**Full Authorization:**
- I/We provide full authorization for Angel Paws USA to take any necessary actions to complete the pet funeral services, including transportation, cremation, and any related services. I/We trust Angel Paws USA to handle all arrangements with care and professionalism.

**Acknowledgment:**
- I have carefully read and fully understood the foregoing provisions and agree to the payment agreement, terms disclosure, and all fees for these services. Electronically Agreeing counts as an original agreement for all purposes. By agreeing below, you agree to the terms and provisions of this agreement.
