I also certify that no guarantee or assurance has been made to the results that may be obtained. I also understand that these services likely require physical contact and that I can refuse any part of treatment that makes me feel uncomfortable. Finally, I understand, in the event of an emergency, it may be necessary to provide services in order to preserve my life or health, and I consent to these services. Licensed members of Divergent Physical Therapy & Wellness are certified in First Aid and CPR by the American Red Cross.
