Dear Parent or Guardian, 

Your seventh-grade child is being asked to be a part of our school’s Healthy Kids Survey sponsored by the California Department of Education. This important survey provides us with valuable information on our continual efforts to provide safe and drug-free, positive learning environments for HBCSD students. We value opportunities for meaningful student feedback and involvement. Research has shown that healthy students learn better. Developing schools that are drug-free, safe havens for positive youth development is central to our mission and ability to teach. Research studies and reviews over the past decade have consistently concluded that student health and wellness status and achievement are inextricably intertwined. Student participation is optional. Please read this form for information about the survey, and for instructions on how to withdraw your child. 

Survey Content- The survey gathers information on behaviors such as physical activity and nutritional habits; drug use; school safety; and environmental and individual strengths and assets. 

Administration-  The survey will be administered March 27-28 and will take about 20-30 minutes to complete and will be administered by your child’s science teacher.

Participation is Voluntary and Anonymous-Your child does not have to take the survey. No names or any other identifying information is connected to the answers except for the name of the school. Students who participate only have to answer the questions they want to answer, and they may stop taking the survey at any time.
