Some of the questions may seem quite personal and you are not legally obligated to answer them. I would however encourage an honest and frank approach and can assure you that your personal information will be used in an appropriate fashion and for the purposes of safety only.

If the new member is under 16 years old or is someone who will be attending with a carer, the form must be completed by the carer or responsible adult.
