Recital 2024 Registration
For information on recital date, time, location, etc... please visit www.eimdance.com/recital. Please submit a separate form for each child. Our costumes are ordered mostly from Target which is why we ask you for their shirt and pant sizes. We will take measurements ourselves for any sizes needed for a leotard/ballet outfit which is ordered from costume companies. Reminder that if your child is between sizes it is better to order on the larger end. It is easier to hem an item than to take something out. If your child can wear a YXL or Adult small please choose YXL. Children who are in more than one dance class a week will be charged the recital fee and $50 extra per additional class.

I understand that my child MUST attend dress rehearsal in order to perform at recital.  NO EXCEPTIONS. *

I understand that if my child's costume(s) fits significantly too small or too big, then I have TWO DAYS from the day I receive the costume to notify EIM Dance to exchange the costume for a size that fits best. NO EXCEPTIONS. *
