Shine Registration Form 2023-2024
Hi there! I am happy you found yourself here! This form is to gather information about both your child interested in our Shine (Preschool - 5th grade) programs as well as your family.

Please note we ask that your Preschooler is at least 3 years old (by September 1st) and is fully potty-trained. 

If you are signing up multiple kids for the same program please fill out a separate form for each child you are registering for our Shine programs.
