Please fill out this form to begin registering your child for baptism within our Blessed Trinity Family (Corpus Christi, St. John the Baptist & St. John Neuman).  If you have multiple children being baptized, please fill out the form once for each child.

After completing the form, we will contact you to speak about formation and scheduling a date for the baptism. Thank you!

GODPARENTS
Godparents must be fully initiated Catholics and active member of a parish.  A non-Catholic, baptized Christian may serve as a Christian Witness in place of one Godparent.  Godparents must be of the opposite sex. 

Godparents who are Catholic must fill out a form and have it signed by themselves AND the priest of the church they are registered at.  That form can be found HERE or can be emailed.
