Behaviour problems exhibited by horses can be very frustrating, and also highly dangerous. When formulating a management plan, numerous factors need to be taken into account. Please fill out this questionnaire prior to your appointment. Information that you provide in the questionnaire will help guide the consultation for accurate diagnosis and will reduce the time needed for history taking, so that your horse's management plan can be developed and explained thoroughly.

When you provide information, please detail the behaviour you see rather than your interpretation of what the behaviour may mean about your horse's emotional state or character.

During the consultation, I will be explaining steps to help your horse. You will be provided with recommendations so that we can work towards a long-term plan.
