Cherokee Families,

This Change in Transportation form must be completed on the day a temporary or permanent change is necessary for your child’s dismissal. Complete a separate form per child.

This form must be completed NO LATER THAN 12:00 p.m. on the day of the change. For an emergency late transportation change after the stated time, please notify the School Office at (847) 234-3805. Thank you!
