Click the 'continue' button at the end of each page and the blue 'submit' button at the end to record your details and a confirmation message will be displayed.  Please fill in the order form first.  Payment can be made by cash (exact money only and no coins) or by PayPal.  Hotdogs and vegan sausages will NOT be ordered for anyone who has not paid by the deadline.

The email address you provide on this form will be used for PayPal billing if you choose PayPal, to contact you with any queries about this order and to contact anyone who says 'yes' to volunteer at the Sizzle Day.

Complete this form and pay no later than 2pm on the 20th of May. We can not guarantee your child will receive a hotdog if you do not fill in the form and make your payment before the deadline. Thank you.
