FBLA MAY DAY RUN REGISTRATION FORM
Ashland-Greenwood will be holding their second annual 5k run/1 mile walk on May 7th. Registration will start at 8:30 AM in the high school parking lot, and the run will start promptly at 9 AM. The entry fee is $30 a person and this will include a race t-shirt. In order to guarantee t-shirt availability, you must be registered by April 20th. Please mail or take your fee to the HS office Attention: Amber Dutcher. (1842 Furnas St, Ashland.) All proceeds will go towards food insecurity in the Ashland area.

May Day Run Waiver: In consideration for being allowed to participate in the FBLA May Day Run/Walk, I (for myself and anyone entitled to act on my behalf) waive and release Ashland-Greenwood Public Schools, City of Ashland, Event Sponsors, and/or Volunteers from all claims or liabilities of any kind arising out of my participation in the May Day Run; even though such claims or liabilities may arise out of negligence or carelessness on the part of any person identified in this waiver. My decision to participate in the May Day Run/Walk is entirely voluntary. I know that running/walking is a potentially hazardous activity, and I represent that I am physically capable of participating in this event.
