Parent Responsibility Agreement
As a parent of the applicant, I understand that this scholarship, if awarded, is half ($12) of the entire cost of a weekly lesson and that I am responsible for the remaining ($12) cost of the private lesson fee. I understand that the private instructor must be notified 24 hours in advance of a conflict, except in case of an emergency. Should the student have an unexcused lesson, I agree to pay the full amount ($24) for the unexcused lesson. Unexcused is defined as there having been no documented communication of an absence a minimum of 24 hours prior to the lesson time. All applicants will have to reapply in August for scholarships for the next school year. Since funds are limited/budgeted each year, I also understand, should my financial status change, I will notify McKamy Band Boosters I will no longer need the (1/2) scholarship so funds might be diverted to another student in the band program.
