Reporting of Total Compensation of Subrecipient Executives
Intended Audience: Financial Officers from any jurisdiction or public institution for higher education in the state of Utah interested in applying for federal grants through the Utah Division of Emergency Management.

Purpose: Federal Funding Accountability and Transparency Act of 2006 requires you to report the names and total compensation of your entity's five most highly compensated executives if other requirements are not met.

Time Commitment: This tool will take approx. 15 minutes. We recommend that you set aside 30 minutes to complete this form before beginning.

Knowingly providing false or misleading information may result in criminal or civil penalties as per Title 18, Section 1001 of the US Criminal Code.
