2021-2022 School Year Release of Vaccination Records
On July 9, 2021, IDPH and ISBE adopted the recommendations from the Center for Disease Control (CDC) regarding guidance for COVID-19 prevention in K-12 schools. These strategies will include masking, social distancing, and availability of on-site rapid COVID-19 testing when students present with symptoms. Students who have received the COVID-19 vaccine are not required to follow mitigation strategies unless they exhibit COVID-19 symptoms.
In order to properly share vaccination status, parents must provide consent for the FVSD Nurse to access Illinois Comprehensive Automated Immunization Registry Exchange (I-CARE) and share vaccination status with authorized staff. Authorized staff include administration as well as staff with direct contact with your student (teachers, coaches, support personnel). You are not required to share access to I-CARE, however this provides the most convenient verification. Should you wish not to share vaccination status of your son or daughter, you child will be treated as an unvaccinated individual for purposes of mitigation strategies. Alternatively, your child may show their vaccination card as proof of vaccine.
