Recognizing a community member for outstanding committee work, performance in office, or execution of event-related duties, during this particular program year. A nominee must be a current SLA member.

Recognizing a community member for a sustained record of supporting community activities over the years. These may be for service on one or more committees, by holding various offices, or for managing different events, that when considered cumulatively have notably furthered the life of the community. A nominee must be a current SLA member.

Conferred on an individual who is recognized as a longtime contributor to the community and the organization. Traditionally this was given when someone "retires" from active community activities and/or the profession. A nominee must be a current SLA member. 

You MAY make multiple submissions. You may nominate a candidate for each type of award, and you may nominate multiple candidates to the same award type. A nominee must be a current SLA member. Fill out one nomination then submit the form. Then you may select "Submit another response" as many times as you need to make all of your nominations.
