Please list the names and contact numbers of any people that your child may be collected by (they must be over the age of 16, so please do not include any siblings under this age). Please also indicate their relationship to your child, and their phone number so that we can contact them if they do not arrive.
Please note that if the person who is collecting your child is not on this list then we will not allow them to take your child, even if we have seen them before. As stated in the revised Early Years Foundation Stage, the staff in your child’s class must be informed beforehand about who is picking your child up.
Please write them in order of who will be the most regular people collecting your child. If it will be specific people on certain days, please write this below. See the example below if you are not sure. It would be helpful if your child knows on a daily basis who is going to be collecting them so they know who to look out for at the end of the day.
If the list needs to be changed then you can either see one of the staff in your child's class to add or take away names or send updated details via a letter to your child's class teacher. Don’t forget to add your own names (mum and dad) and telephone numbers. Please note that each parent with parental responsibility will automatically be allowed to pick up but please write yourselves on this form.
There is space for 10 names here, but don't feel you have to have that many people on your list! However, you may find it easier to have more names than you think you will need.
Your child's class teacher will print off this list so that there is always easy access to it at the end of each day.
