I, as the parent or legal guardian of the below Student, give my permission for my child to participate in the  event listed below at the below location on the date listed below. Parents/Guardians must provide transportation to and from the event, or I understand that my child will travel to and from the event by means of the transportation listed below. This permission includes all related programs or activities associated with the event. In consideration for my child’s participation, my child and I agree and understand that we assume any risks inherent with this event and with full knowledge of the risks, I agree to release and hold harmless the Director of Youth Ministry and any of his agents, St. Albert the Great Parish, and the Archdiocese of Philadelphia and their employees and representatives, from claims arising or related to my child’s participation. My child understands and agrees to abide by all rules and regulations established by St. Albert the Great Parish pertaining to this event, and I have spoken with my child about his or her responsibility to act appropriately at all times.
