Hello and thank you for taking the time to provide us with your dog's history to help us ensure they are a good fit for our playgroups. Please note we currently have a waitlist for our playgroups. 

In addition to this form, please submit Vet documents from your local Vet to amyterceira@gmail.com, stating they are up-to-date with vaccines and have been dewormed within the last 6 months.

If you have more than one dog, please fill this out as many times as needed to include all the dogs in your household.

If your dog struggles with fear, aggression, or reactivity, playgroups are not the right place to start, private training is. Please fill out the intake form on the behavior consultation page.

Please note that your dog can not attend playgroup if they are in heat, have loose stool, are vomiting or coughing or have fleas or any contagious skin disorder. Please seek veterinary assistance.  Playgroups are for friendly dogs who do not suffer from fear, aggression, or play skill deficits.
