End of Year Report - 2021-2022
Thank you for being a part of Westgate  PTA.

The current/outgoing chair should submit the following form due each spring or when your event/fundraiser is finished. . Keep a copy for the committee file. If you are outgoing, please ensure your successor receives all committee material or drop them off with the President or VP.  

All committees also need to complete a Plan of Work Report before the end of the fiscal year or when their committee work is finished.
