Informed Consent

This research is being conducted by Christopher L. Young. A Senior at Cleveland State University, perusing a bachelor’s in psychology.

This study will look at the self-esteem of weight loss and bariatric surgery patients and will examine how large amounts of weight loss (75 pounds or more) affects their self-esteem.  You will be asked to complete 2 different surveys. The first is a 10-item survey asking you to rate each question on a scale from 1 (Strongly Agree), to 4 (Strongly Disagree). The purpose of this is to study your current self-esteem. The second is a 35-item survey asking you to rate each question on a scale of 1(Strong Negative Feelings), to 5 (Strong Positive Feelings).  The purpose of this is to study your self-esteem prior to your weight loss. Also, we will study the difference in self-esteem between males and females.

You will spend no more than 15 minutes answering questions. You are not required to participate in this research. You may stop at any time. Your answers will be confidential. We will take numerous measures to ensure this confidentiality. Your name will only appear on this informed consent form. Data will only be presented, published, or discussed only in group form. Any risks associated with this study are minimal. They are not beyond that of daily activities. There are no risks, discomforts, or inconveniences anticipated in this research. Your data are important to this project. We hope that your participation informs you about psychological research. You must be eighteen years are older to participate.

Participation is completely voluntary. You may withdraw at any time without penalty. Should you choose to withdraw, the information you provided up to that point will be discarded.
