2020 Convention Gala - Dietary Restrictions
The 2020 Convention Gala takes place on Saturday, March 28, 2020 at the Convention Hotel. You must purchase a ticket in advance ($25 for registered attendees, $30 for guests) in order to attend this event. You must select either a vegetarian or chicken entrée when registering; however, if you have dietary restrictions and/or allergies please fill out this form. We will give this information to the chef preparing the meal so that they can be sure your meal will meet your dietary needs.

1) When you check in at the on-site registration desk you will receive a ticket for the gala.
2) On the back of the ticket will be a label with the information you provide in this form.
3) You must bring this ticket to the gala in order to receive your meal at the gala.
5) Please look at your ticket when you check in to make sure that the label attached to your ticket is accurate.
6) If it is not attached, let the registration desk know immediately.

You will receive a confirmation email after you submit this form. If you don't receive a confirmation email, contact the Central Office at englishconvention@niu.edu or 815-981-9974.

This form must be completed by MONDAY, MARCH 16, 2020 in order for Sigma Tau Delta to accommodate your request.
