Mental Health & Safety Parents / Adults Survey
Thank you for taking the time to take our survey! We are the Daly City Bay Leaders, we are a group of youth that live in Daly City who come together to talk about issues that affect our community. We believe that youth have the power to make changes in our community and that we are not just future leaders, but leaders today. We know this year has been filled with so many changes for everyone, and it is important for us to be engaged in what is happening around the community. We want to learn more about how our fellow peers feel about safety and mental health resources in their schools.

This information will be used to educate teachers, the School Council, the Jefferson Union High School District School Board, and the City Council about what is needed and the experiences youth are facing when it comes to safety. This data will be used to create recommendations that are youth-led to help address these concerns. We hope to ultimately have a safe community along with knowing where the gaps in resources are.

For a chance to be entered in a raffle for 1 of 3 $30 gift cards have your parent/guardian fill out the Parent/Guardian survey at
