INSTRUCTIONS:

Thank you for registering your child in LIKHA School of Dance. Children must be at least 5 years of age at the time of registration.

To allow for social distancing, we will be limiting registration to 15 students, first come first served. Per the City of Oakland, anyone entering the studio aged 12 and older must show proof of COVID-19 vaccination. All people entering the building (regardless of vaccination status) will be required to wear a mask. We will require all instructors and students (regardless of vaccination status) to wear masks during the class. Parents may drop off their child but will not be allowed in the studio.

This session will be offered free of charge, however if you would like to support LIKHA in holding classes like this in the future, the suggested donation per family is $125. Our donation link can be found at likha.org/donate.

**If you are enrolling more than one student, please submit a separate registration form for each child.**

Closer to our start date, you will receive a link to complete a liability waiver for each student via DocuSign.
