2022 SunBurn Art Registration
This Art Registration form consists of 4 main sections: Art Registration, ART Installation and Removal Plan, Art Burn Plan, & Art Grant Application. Depending on your answers to certain questions the form may have you skip the Art Burn Plan and/or the Art Grant Application. Please give yourself around 30 min to complete if you are filling out all sections.

In order to keep this form available to folks who do not have a Google account, we are unable to use the Google Forms' file upload features. In Art Registration, we request an email of some sort of depiction, illustration, sketch or photograph and in the Art Grant Application, we request an email of your budget documentation. We would prefer these be combined into a single email with the subject line: "<your art project's name> Documents", unless email attachment size prohibits this. Keep in mind most email servers will only upload 25mb files. We can also send you a Google Drive share link if email attachments are not an option. Email us a request for a Google Drive upload link if you need it. Please email all requested documents to art@nmsunburn.com.

The Art Registration sections requests some detailed information. Feel free to put any response in to fulfill the "required" requirement and submit the form if you wish to fill these in during multiple sessions. There is an 'edit my response' link at the end of this form as well as in the copy of your form responses that is automatically sent to you.
