To withdraw from lessons, please complete this request form. A minimum of 15 days notice is required to withdraw from lessons. Once this form is received, a CMS staff member will contact you at the email address provided to confirm the cancellation of your lessons and billing plan. Upon submission of this form, CMS will start to process your withdrawal. You will still be expected to attend/pay for any lessons that fall within the 15-day notice period. If you will not be attending any further lessons, please notify CMS. If you do not wish to withdraw immediately, please request your final lesson date in the "Other" section:
