CMS Lesson Withdrawal Request Form
To withdraw from lessons, please complete this request form.  A minimum of 15 days notice is required to withdraw from lessons.  Once this form is received, a CMS staff member will contact you at the email address provided to confirm the cancellation of your lessons and billing plan.
