ADULT PARTICIPANT / LEGAL GUARDIAN CONSENT: By participating or allowing my child/student to participate in the Cat Packs Down & Dirty *I am aware that photos and video taken at this event can be used to promote it. *I understand that all reasonable safety precautions will be taken during the event or activity.    * I understand the possibility of unforeseen hazards and know the inherent possibility of risk. *I agree not to hold the Dallastown Cat Packs Organization, the Dallastown Area School District, its Board of School Directors, its officers, employees and volunteer staff liable for any damages, liabilities, losses, penalties, costs, expenses, legal fees, claims or injuries incurred or resulting from the activity of Cat Packs Down & Dirty. *I understand that the registration fee is a donation to Dallastown Cat Packs and will NOT be refunded for any reason.  Refunds will not be given if a permission form is not signed or if a student is unable to attend. *
MINOR CONSENT: As a student who is participating in the Cat Packs Down & Dirty, I accept the responsibility of complying with the disciplinary rules contained in the District’s policies and the Student Handbook. I also understand that I, or my entire team, may be removed from the event at any time if my/our conduct fails to meet those standards.
