2021 40th Anniversary Gala Table Host

This form is for those who are interested in hosting a table at our 40th Anniversary Gala on November 4th at The JW Marriot in Downtown Grand Rapids. Hosts will invite their family and friends and are expected to fill 7 seats per hosted table (6-8 seats including themselves).

After you have signed up to be a table host, The Potter's house will send you invitations to give to your desired guests. Guests will then RSVP themselves online, and select you as their table host.

There are two options for what time you've like to host your table:
Lunch gathering (Registration at 11:30am, lunch begins at 11:45pm)
Dinner gathering (Registration at 6:00pm, dinner begins at 6:30pm)
