In signing this application or by paying an entry fee and fishing the Dynamic Marine Bass Trail LLC, I hereby release the host, Dynamic Marine Bass Trail LLC, its owners, employees, sponsors, and tournament officials from any and all damages, claims, demands, cost or expenses relating to injury of any persons or damage to any property which I sustain or which I may cause by reason of participating in or in connection with this Dynamic Marine Bass Trail LLC tournament.

I further agree that I will never sue any of the above for damages on account of any injury I suffer or cause, whether known now or which may develop in the future, in connection with the Dynamic Marine Bass Trail  LLC tournament. In the event that any of the above are sued because of my actions, I expressly agree to indemnify and hold each harmless from any liability whatsoever, including court costs and attorney fees arising with respect to such actions.

By signing this form and/or paying an entry fee, I agree to submit to all rules set forth by the tournament committee and the tournament director. I understand and agree that the tournament director or the tournament committee reserve the final decision in any situation and that I agree to submit to a polygraph examination at any time.

By signing this form, I hereby authorize this organization to use my photo, video, and likeness for the purpose of promotion in news releases, photographs, video, audio, website, marketing and advertising for an indefinite period of time.
