The information you provide on this form will be vital to the most important aspect of our program, parent/teacher/student communication. I use Charms Office Assistant to handle, organize, and manage day-to-day operations of the the orchestra program, including communications via e-mail, text, and phone messaging with students, parents/guardians, volunteers, and chaperones. I also manage trips, fundraisers, and financial transactions in accordance to CCPS policies, as well as physical inventory items such as sheet music, instruments and accessories. Parents and students have access to their own accounts to keep up-to-date on important orchestra events and information. 

Please fill out this form for each child you have in orchestra. A copy of your responses, as well as the information contained in this form (materials, recommended places to get instruments, etc.) will be sent to the e-mail you provide immediately below.
