LIABILTY WAIVER; U.S. Figure Skating, CHFSC, Cottonwood Heights Learn-to-Skate program and the CHRC under take no responsibility for damages or injuries suffered by skaters or officials. As a condition of and consideration of the acceptance of their entries or participation therein, all entrants, their parents/guardians and officials will be deemed to agree to assume all risks or injury to their person and property resulting from, caused by or connected with, the conduct and management of the competition, and to waive and release any and all claims which they may have against any officials, U.S. Figure Skating, CHFSC and its officers and volunteers, Cottonwood Heights Learn-to-Skate program and its coordinators and staff, and the CHRC and its officers, management and employees. Release & Indemnification: I hereby recognize and acknowledge that myself and/or my child's participation in this activity may involve bodily injury and/or emotional injury to myself and/or may child. In consideration of myself and/or may child being permitted to participate in such an event. I for myself, my child, my heirs, my executors and administrators, hereby voluntary and knowingly indemnify and hold harmless, defend, release, waive and discharge U.S. Figure Skating, CHFSC and its officers and volunteers, Cottonwood Heights Learn-to-Skate program and its coordinators and staff, and the CHRC and its officers, management and employees. In addition, I agree that I or my insurance company will pay for medical, hospitalization or any other expenses resulting from myself and/or my child's participation. Emergency Treatment: I hereby authorize CHRC program staff to act on my behalf in accordance with their judgment in case of an emergency involving myself and/or my child, and agree to assume full responsibility for all expenses, medical or otherwise, that may arise therefrom. I understand that I or my insurance company will pay for such emergency treatment. By agreeing to this waiver of liability, I acknowledge that I have read its contents and disclosure, that I understand its content and s, and agree to its terms. I also agree that my participant(s) and I will comply with current health guidelines as established by U.S. Figure Skating, Salt Lake County, the State of Utah, Cottonwood Heights City, and/or Cottonwood Heights Parks and Recreation, as applicable.
