By signing this form, I agree to abide by the PFA’s Constitution at all times, including upholding the values in clause 2 of the Constitution. I am aware that my registration needs to be renewed at the next Annual General Meeting (usually held in October).

In order to operate and fundraise, the PFA must have declared office bearers, please indicate which position you may be interested in nominating for? There is always the possibility to co-share a role, i.e co-presidents, co-treasurers to share the load. *

Just because you check the box, doesn't mean that you will become or are self nominating for this position. It's simply a point of conversation if we can't fill the positions.

President (primary role is to make sure all events have someone managing them and/or step up if needed,, make sure treasurer and secretary are doing their roles)

Treasurer (primary role is to make sure any cash received is counted per school policy and fundraising balances are passed on to PFA team)

Secretary (primary role is minute-taker for agenda meetings, provide content for newsletter)

Privacy Information

The PFA Secretary is required to maintain a log of registered members of the PFA and their addresses in accordance with the PFA’s Constitution. The PFA will seek to provide information including meeting agendas and any supporting documentation via email.

This form will be filed in the school building, will be the property of the Minister and will be made available to officers authorised by the Secretary of the Department of Education and Training.
All school clubs, (including Parents’ Clubs), are governed by the Department of Education and Training, and therefore must protect the information that they collect.
