How did you hear about Avalon?

The undersigned hereby submits a membership application for Avalon Recreation Association, Inc. and agrees that, if accepted, will abide by all of the bylaws, rules and regulations of the Association and any and all bylaws, rules and regulations that may be promulgated in the future. The undersigned also agrees to pay the Initiation Fee and the Annual Dues in effect. The undersigned acknowledges that the Board of Directors has the right to change the Initiation Fee and the Annual Dues prior to the approval of this membership application.

Finally, in consideration of being granted membership, the undersigned agrees, on behalf of himself, herself and their minor children (if applicable) that Avalon Recreation Association, Inc. may use and publish their names and photographs in the publications, newsletters, directories and websites.

Now therefore, I, or we, the undersigned, agree jointly and severally to pay the Initiation Fee and Annual Dues.
