Field Day Consent Form

Hey Parents we are getting things ready for our annual Field Day at our main campus. This year our PTO has graciously donated 1 meal to every student which will include a hotdog, bag of chips, and a water bottle. If you would like to purchase an extra meals or a shirt, please use the links on the form to order through our online shop. The deadline for ordering shirts is April 23rd. The deadline for extra meals are May 1st. This form gives your children consent to participate in field day. You can also sign up to volunteer with it as well. Our dates for Field Day this year are: 

May 6th, 9:00-2 pm - Kindergarten and 1st Grade
May 7th, 9:00-2pm - 2nd Grade and 3rd Grade
May 8th, 9:00-2 pm - 4th and 5th Grade
