STUDENT AGREEMENT FOR 2021 UNIVERSAL STUDIOS GRAD BASH ACTIVITY
As a senior student/parent of a senior student in good standing (academic and citizenship GPA’s of 2.0 as of April 1), I agree to the following to ensure safety and fun for all:

I would like to participate in the PTSA sponsored senior activity at Universal Studios from 12:00 PM Friday, May 28, 2021 through approximately 5:00 AM Saturday, May 29, 2021.

I agree to adhere to all rules and regulations listed below and understand that failure to follow the rules or behave as an excellent representative of my senior class will result in my removal from the activity and forfeiture of all money paid.

PTSA and all adult chaperones reserve the right to refuse participation to any student who is found to have broken any of the following rules: including, but not limited to, fighting, suspensions, vandalism, or bringing alcohol, drugs or weapons to the school campus, on the bus, or to Universal Studios. Behavior problems between now and the date of the Universal Studios activity may result in my removal from the activity and forfeiture of all money paid.

Anyone found to be or suspected of being under the influence of alcohol or any other controlled substance will not be allowed to board the bus to Universal Studios and will be sent home with parents. Breathalyzers will be available to the security staff. Participants are rechecked by Universal Studios staff prior to entering the park and may be refused entrance. (Students who are refused entrance will be escorted to a holding area and parents will be notified to come to Universal Studios to pick them up – they will not be allowed to ride the bus back).

Students will not be allowed to bring any food or drink on the bus. Universal Studios will not allow food or drink to be brought into the parks, so please plan to bring money for food purchases.

Students must arrive at the departure site for the buses promptly. We will not wait!

Students must have their school ID to board the bus.

Large backpacks or large bags of any kind will not be allowed into Universal Studios.

All carry-on items and clothing are subject to search prior to boarding the bus.

Written information about any medications needed during the trip must be provided to the bus chaperones. Medications should be in clearly labeled, original containers and will be held by chaperones upon entrance and returned once in the park. Students may carry inhalers.

There is no smoking/vaping at Universal Studios or on the buses.

Any item that could be used as a weapon is forbidden and will be confiscated and discarded.

Students must travel to and from the Universal Studios activity on the PTSA chartered school bus.

Students may not leave Universal Studios until the main gates are opened and they are released to return to the buses.

I have read, understand and agree to all policies and restriction of Universal Studios as explained on their website (https://www.universalstudioshollywood.com/web/en/us/policies-and-restrictions)

I assume the risk in my student’s participation in the event. I acknowledge that I will not seek to have the PTSA and its volunteers held liable in the event that any accident, injury, loss of property or any other circumstance or incident occurs during or as a result of my student’s participation in this trip. This release of liability includes accident, injury, loss, or damages to the student, as well as, to other individuals or property, which may result from the student’s participation in the event. I hereby release and agree to hold harmless the School District, the PTSA and the volunteer chaperones, from any claims arising out of my son’s/daughter’s participation in the event.  

My parents and I accept financial responsibility for any property damage I should cause. This includes, but is not limited to, damage to the school buses.
