Taft 7-12 Disc Golf Sign Up Form 2024
Disc Golf is a club activity for 7th -12th graders Taft 7-12 students to participate in. Practices will be during the week after school April 1st-May 11th. The season goes April 1st to May 11th.

Events (Twisted Pines DGC South Beach State Park):
Family Field Trip - April 11th 3-5pm
Student Meet - April 25th 3-5pm
Qualifier - May 4th 10am-6pm
Final Tournament - May 11th 10am-4pm 

There is no cost and no sports physical required as this is a club activity and not a sport.

As parent or legal guardian of the Student named, I hereby give consent and permission for my Student to participate in the activities selected previously. I certify that I have legal authority to waive, discharge, and hold harmless the released parties on behalf of the Student. I understand that the activity is voluntary and I am fully aware of the possible risks and injuries. My Student agrees to follow the LCSD Student Code of Conduct (Policy JFC-AR). I understand that my student must participate in the entire activity including school-sponsored transportation unless previous arrangements have been made with the principal.I hereby release Lincoln County School District and its employees, Board of Directors, volunteers, and representatives from all liability for any claim that I or my student may have for injuries, damage, or loss. Both student and I voluntarily agree to waive and discharge any and all claims against Lincoln County School District and release it from any liability regardless of cause for the activity selected previously. *
If applicable, my student has permission to be transported in a private vehicle during this activity. *
