If you normally have funding or employment in the summer that has been disrupted by COVID-19 and you have not been able to find replacement funds, please let us know by completing this form. We mean all income LOST not just that from the university. So, if you normally work summer stock or had an internship or do gig economy work or teach at a summer school off campus, that should all be included. For example, if you have a partial fellowship from your department and are still getting that but you have lost your part-time side job that helped supplement, we want to know about that side job.

The more information we have about overall losses, the better planning we can do. If you are still going to receive the funding you were promised for the summer, have an internship that is still happening, and so on, please do not complete this form.

Please note this is not a request for summer funding. This is just a request for information at this point with a focus on those who have no summer funding or who have lost chunks of their funding.
