Deposits and Fees
Please submit this form with your $500 (Preschool) NON-REFUNDABLE AND NON-TRANSFERABLE deposit. You will owe a second deposit on May 15, 2024 of $500 (Preschool) / $750 (Gan). 

When you receive your full tuition bill, the amount due will have been reduced by your deposit amounts. Your application will not be processed until we receive your first deposit and your registration will be cancelled if we do not receive your second deposit.

A $50 registration fee is billed per student
A $250 security fee will be billed per family per year
A $180 HSA fee will be billed per family per year

I will need lunch care 12:00pm - 1:00pm (for half day students only) 4 or 5 days a week is $1800 for the year. One day per week is $475 for the year. (There is a daily option of $15 per day but MUST know  the day before) (THIS IS NOT HOT LUNCH)
I will need Extended Day 12:00pm - 3:05pm (for half day students only) One day per week is $1050 for the year. (There is a daily option of $50 per day but MUST know  the day before)

Any additional information you would like us to know about your child? (Parent requests for specific placements cannot be honored. We ask your cooperation in refraining from making such requests)
The Noreen Cook Center has our permission to use any photo, picture, or likeness of our child or any family member for promotional purposes, including the Har Zion website, school website and Facebook page.
Application Requirements & Financial Arrangements:
HAR ZION MEMBERS: In order for us to process your child's application for the 2024-2025 school year at the member rate, your 2023-2024 membership dues must be paid in full at the time this application is presented to the office. Furthermore, parents enrolling their child in the Noreen Cook Center as members must maintain a valid membership throughout the school year. Therefore, it is understood that if accepted, your 2024-2025 membership dues and tuition bill will be paid in full by August 15, 2024 unless automatic payment arrangements are on file. If membership renewal is not made as noted, school fees will be converted to the Non-Member rate retroactively. All requests for financial assistance should be made immediately to the Noreen Cook Center Director.
TO ALL FAMILIES:
This application can only be processed if all school and other 2023-2024 synagogue obligations are current. We look forward to having your child with us in the preschool. Occasionally a child may be asked to leave because the child is not ready for school, has a special problem that cannot be handled within the limits of a normal classroom situation or otherwise because of our concerns for the safety and well-being of the class as a whole. Tuition previously paid will be pro-rated and refunded accordingly.
This application is a legally binding contract. All financial obligations owed to Har Zion Temple for the above named child are the responsibility of both parents. This application will not be processed unless both parents acknowledge this financial obligation by signing below. Any exceptions or special circumstances require the prior written approval of the Noreen Cook Center Director. We have read and agree to be legally bound by all of the terms and conditions set forth above. *

If you select to use a credit card: All Credit Card payments will incur a 2.5% processing fee added to each transaction. I understand that I must notify Har Zion Temple when my credit card expires or is cancelled and supply them with an updated card number. If credit card is declined, a replacement must be given within 15 days or a fee of $25 will be assessed on my account. Please contact our Bookkeeper Rhoda Eisman at reisman@harziontemple.org to arrange a time to collect your credit card information.
If you select to use your bank account - ACH Checking Account:  Please contact our Bookkeeper Rhoda Eisman at reisman@harziontemple.org to arrange a time to collect your banking information.  I understand that I must notify Har Zion Temple four (4) days prior to the 1st of the month if my checking account number changes and provide new banking information.  If an ACH payment is declined for any reason a $25 fee will be assessed on my account.

* CANCELLATION & REFUND POLICY: No refunds will be issued and/or credit transferred to any other division of Har Zion Temple for any reason unless adjustments need to be made. Exceptions to this policy can only be made with written approval by the Executive Director.  Please note that if there are any special circumstances or problems of which the school should be aware, please contact our Noreen Cook Center Director before this application is filed. This application is a legally binding contract.  If HZT determines, in its discretion based on circumstances that school cannot run during the school year, a full refund will be given.
