Permissions/Liabilities/Releases *
PLEASE READ CAREFULLY: By entering the parent/guardian full name below, I hereby grant permission to SMARTgirls/GfG representatives, to take and use: photographs and/or digital images of my child for use in news releases and/or educational materials as follows: printed publications or materials, electronic publications, or Web sites. I agree that my child’s name may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All negatives, prints, digital reproductions shall be the property of SMARTgirls. I grant SMARTgirls representatives permission to use my email for communication, survey and informational purposes and acknowledge that my email will not be released to other parties without my permission. I give permission for my child to attend the camps selected above. I also acknowledge that I will arrange for transportation to and from camp each day. It is also understood that Wylie ISD does not provide medical insurance covering injuries of any nature incurred during the SMARTgirls/GEARSforGUYS camps. The undersigned hereby releases Wylie ISD from any and all claims, demands and causes of action whatsoever in any way growing out of or resulting from participation in any of the camps.
