Registration & Contract
The Sunrise Program is designed to build positive relationships, promote student achievement, and provide a safe environment before school starts in the morning. Our partnership with Nita M. Lowey 21st Century provides opportunities such as this for students enrolled at La'ie Elementary School. Students can check in anytime from 6:45-7:40 am and they don't need to come every day to stay enrolled.
 
WHO:  Students enrolled at La'e Elementary School 
WHEN:  Daily . .  check in starts at 6:45 am. Students can come anytime between then and 7:40 am.
WHERE: Students check-in outside the Laie Elementary Library to self-select activity for the morning.
WHAT:  

  • Quiet Reading, Computers  & Homework Help from teachers

  • Outdoor Activities supervised by teachers

  • Arts & Crafts planned by teachers

  • NEW: Chess Club facilitated by teachers - will start in January.

Please complete this registration form for each child that you would like to enroll in the Sunrise Program.

This form must be completed before your child can start. You'll receive a confirmation email from Courtney Fermantez.

We are partnering with the 21st Century Community Learning Center to provide the Sunrise Program. By submitting this form, you agree to the following guidelines:

Video & Photo Permission: I allow my child to be photographed or videotaped during the course of this program. These photos or recordings will be used solely for the purposes of documentation, evidence of achievements, for accounting and reporting purposes, promoting or celebrating the program.

Attendance & Behavior Expectations: I understand that my child must follow all school and DOE rules.

Contact Information: I will notify the school of any change to contact information to ensure prompt notification.

Data Collection and Evaluation Assurances: I understand that by completing this enrollment form and clicking below, I agree to accept all policies and procedures of the program.  In addition, I authorize my child’s school to release information such as achievement and attendance data for use in planning program activities and evaluating program effectiveness. (Individual students/families will not be identified by name and this information will not be shared with third parties.)

I understand that La’ie Elementary 21st Century Program reserves the right to cancel an activity with prior notice.
