B&C FES2022 試乗申込書＆誓約書

BIKE&CAMP 2022 の試乗車に乗る際の電子誓約書になります。

試乗される方は全員もれなくご記入お願い致します。
なお、回答は修正できないため、誤って記入してしまった際は、リロードしていただき、再度送信をお願いします。

※ご記入をいただいた方のみ試乗できます、最後に回答のコピーを自分宛にメールするにスライドチェックをして、そのメールが試乗チケットとなりますので、必ずお控えください。

飲酒、酩酊状態での試乗は固くお断りします
注意事項を遵守していただけない場合は、試乗をお断りすることがあります
また、ヘルメット、サイクルグローブをお持ちの方はご持参ください。

※無い方は現地で軍手を販売致します。
ヘルメットはレンタルがありますが寄付金をいただくことをご了承ください。

試乗中、イベントへの道中、イベントでの怪我等については当イベントでは責任を負えないため、1DAY保険などにご加入いただくほうが安心かと思います。

個人情報の取り扱いについて
この誓約書に記入された個人情報は自転車キャンプツーリズム協会が管理し、本イベントに関する事案のみ使用します。お客様の個人情報をお客様の同意なしに業務委託者以外の第三者に開示、提供することはありません。（法令などにより開示を求められた場合を除く）

私は、試乗自転車をコース以外では降車し、手で押して移動します。借りたときと同じ状態で借りた場所に返却致します。I get off the test bike outside the course and push it by hand. We will return it to the place where you rented it in the same condition as when you rented it. *

イベント内で主催者ならびにメディア関係者が行った撮影において、画像・動画については自由に使用することを認め、肖像権などを請求致しません。（※写真が苦手な方はステム位置にバンダナ等の目印を付けてください）
We allow the free use of images and videos taken by the organizers and media personnel during the event, and we will not claim portrait rights.
