Membership Application Form

Membership Types
1. Active: Members who are currently employed on a full or part-time basis by a school district in Ohio and has among his/her responsibilities some segment of the administration or coordination of the interscholastic athletic programs of that school or district.
2. Retired: Members who has been an athletic administrator on a full or part-time basis by a school district in Ohio and has among his/her responsibilities some segment of the administration or coordination of the interscholastic athletic programs of that school or district.
3. Allied: Members who do not meet the aforementioned criteria, but are interested in supporting the Association.

Membership Terms: The annual dues for all M.A.D.A. members are due on August 1. Membership will begin on August 1st and end on July 31st annually.

(NOTE: Dues subject to variation annually based upon number of Members, and expected M.A.D.A. expenses asagreed to by all voting Members of the Association)

ACTIVE MEMBERSHIP COST= $150.00
RETIRED MEMBERSHIP & ALLIED MEMBERSHIP COST= $75.00

MEMBERSHIP BENEFITS: All paid members of MADA will receive the following annually:
1. 1 vote in all business matters directly related to MADA
2. Meals provided at all regularly scheduled MADA meetings
3. 1 meal provided at OIAAA
4. One piece of MADA branded apparel
