In accordance with Governor Cuomo's Executive Order, the Putnam Valley Central School District is gathering information to ascertain if we have first responders or essential healthcare professionals in our community who need assistance with childcare for students, Ages 5-12. Please fill out the following survey if you believe you meet these criteria and require childcare during the weekday between the hours of 8:00 am and 6:00 pm and a representative from the school district will be in touch with you.  In the event that you believe you meet these criteria and are unable to complete this form, please contact Jeannie Denike (jdenike@pvcsd.org).  Please complete this form by Monday March 23, at 4pm.
