2024-2025 Cardington-Lincoln Local Schools Open Enrollment Application
           Beginning at 8:00 a.m. on Monday, April 3, 2024 and continuing through 3:00 p.m. on August 1, 2024, completed Open Enrollment applications will be accepted at the office of the superintendent located at 121 Nichols Street, Cardington, Ohio. When this electronic form is submitted it is time and date stamped. Open enrollment applications are accepted for grades kindergarten through twelve and must be completed annually for each child [One application per child] per the District Open Enrollment Policy. Important: Decisions on enrollment and withdrawal may only be made by the parent(s)/guardians who have residential custody of the child.  At the time of initial enrollment, you must provide proof of the most recent legal documents that verify matters of custody. This information is required by State Law (ORC 3313.672) and the Missing Children’s Act.  Absence of this documentation signifies that both natural parents have residential custody.
