Please complete the entire packet once for each child. You must complete and submit the entire form or your submission will not be accepted. Once you submit this form, a submission receipt email will be sent to your email.

Free & Reduced School Lunch Application

If you would like to applied for Free & Reduced School Lunch, click on the following link to download and print the application. This form will need to be sent to Vanguard prior to receiving free or reduced lunch. Only one form is needed per household.

School pictures will be taken at Vanguard Academy on the first day of school by Lighthouse Printing. Order forms can be printed by clicking on the link below. Scroll to the bottom & click A.9 Order Form Vanguard School Pics 2020. Send completed forms with your child on the 1st day of school.

Vanguard yearbooks and other supplies may be purchased from Vanguard's main office. Contact Evelyn England at 801-327-8724 to make any purchases.
