I hereby apply for membership in Korean Youth Honor Society, and affirm I have read and understand the qualifications for membership. I agree to abide by the rules, regulations, code of ethics and to promote the organization’s objectives and purpose.
Membership may be cancelled at any time with a written notice. I am aware of any risks associated with this volunteer activity and I agree that Korean Youth Honor Society will not be responsible for any injury or damage to my person or property while I am engaged in duties for the organization.
If my works create any problems, including copyright issues, I will be responsible for them. I also understand that the organization may terminate my membership at any time for any reason.
Unless I indicate otherwise in writing, photographs, video tapes, or audio tapes may be taken of me during the course of the activity for use by Korean Youth Honor Society for publicity purposes. My full name and other information about me could be released in the use of the media.
Also, I will pay application fee (not refundable) and monthly membership fee (fixed annually).
