I hereby request to use the Kinder Care service at Das Rec, New Braunfels Recreation Center. I am aware of the policies and procedures put in place to ensure the safety of my child and the Kinder Care Staff. I am aware that there is a maximum time limit of two hours per visit.

For and in consideration of the receipt of the above-mentioned childcare service, I agree to forever expressly release, indemnify and hold harmless Das Rec, New Braunfels Recreation Center, employees, respective agents, and invitees of any and all claims, suits, demands, rights or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf. I also, forever expressly release, indemnify, and hold harmless Das Rec, New Braunfels Recreation Center, the city of New Braunfels, and their respective agents, employees, and invitees of any claims, demands, rights and causes of action whatsoever and unknown, foreseen and unforeseen, bodily and personal injuries, damages to property and the consequences thereof arising out of said childcare service.
