1. Opłatę za szkołę należy uregulować do 31 października bieżącego roku.

2. Zostałem/łam poinformowany, że do przedszkola przyjmowane będą dzieci od 3 lat.

3. Zobowiązuję się do zapłaty $25 za każdą rozpoczętą godzinę opieki nad dzieckiem, jeśli spóźnię się odebrać moje dziecko ze szkoły lub lekcji religii.

4. Zgadzam się na udzielenie wszelkiej pomocy, również medycznej, mojemu dziecku w przypadku wypadku losowego, oraz oświadczam, że w pierwszej kolejności skorzystam z mojego prywatnego ubezpieczenia by pokryć koszty wynikłe podczas tegoż wypadku.

5. Zwalniam od odpowiedzialności za moje dziecko wszystkich pracowników szkoły w przypadku, gdy opuści ono samowolnie budynek szkoły w czasie zajęć szkolnych.

6. Wyrażam zgodę na upublicznienie moich danych/danych mojego dziecka (imię i klasa) na stronie internetowej szkoły oraz na umieszczanie fotografii moich/mojego dziecka na stronie www.pshsienkiewicza.com i Facebook®.
Additional Information
1. Tuition must be paid in FULL by October 31st of the current school year.

2. I have been informed that students will be admitted into preschool as of age 3.

3.  Parents/guardians who are late picking up their child/dren from school, are charged $25 per hour per child at the start of each hour for after-care.

4. I give permission for school staff to provide and arrange for my child to receive medical attention in case of a medical emergency and I agree to use my private medical insurance to cover the costs of medical care.

5. Faculty, staff or volunteers will not be held responsible or liable for any student or his/her actions should the student leave the school premises without teacher's/principal's permission.

6. I give the school staff and volunteers permission to take and to post my child's photographs with limited identifying information (first name & grade level) on the school's website and on social media (www.pshsienkiewicza.com and Facebook®.)

There is a mandatory, non-refundable registration fee of $100  that will go toward the total cost of tuition. Please pay the registration fee within 2 weeks of submitting the registration form.
