NMMS is looking to provide 6-8 grade students a chance to be active, perhaps hone their skills or even learn new ones. Starting date for these skills will be Monday 9/21/20. Times TBD.

                               Newfound Athletics Release of Liability and Indemnification
               Assumption of Risk, Release, Indemnification and Medical Clearance *Phase 3*

As the parent/ legal guardian I authorize my child's full participation in Newfound Memorial Middle School Drills & Skills program that run through the Newfound Area School District. I understand that there are inherent risks for injury and/or illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) through participation in these voluntary events and waive, release, discharge, indemnify, and covenant not to sue the Newfound Area School District and all staff employees/volunteers for my child's participation in the program and/or while upon the premises where the activity is being conducted. My child is covered with family insurance in the event of an accident or injury. I attest my son/daughter is in good health and has no restrictions to participating in summer workouts.

Athlete Responsibility
I agree to follow all instructions and procedures given by the instructors in order to maintain a maximum level of safety. I understand the Newfound Area School District is not responsible for any loss of personal effects brought to any events.

COVID-19 Guidelines
I agree and understand there are specific guidelines in place for athletes to participate this fall and these guidelines may not prevent athletes from contracting or coming in contact with someone who has COVID-19. All athletes will be given instruction on these guidelines prior to training.
