The Parent and Student hereby:
(1) understand and agree that the participation in activities is voluntary and is a privilege.
(2) I/we agree to pick up our athletes at the designated time after practices and contests. Failure to do so may result in the athlete not being able to participate.
(3) I/we understand practices are closed to parents and also understand that because of limited gym space, we will not bring other students/non family members to contests or practices.
(4) I/we understand and agree to a replacement fee of $20.00 on school jerseys if lost or stolen.
(5) I/we understand coaches will not be allowed to give athletes a ride under any circumstances.
(6) Realize that participation involves the potential for injury which is inherent in all sports. I/we acknowledge that even with the best coaching, use of the most advanced protective equipment and strict observance of rules, injuries are still a possibility. On rare occasions these injuries can be so severe as to result in total disability, paralysis, or even death. I/we acknowledge that I/we have read and understand this warning. In case of injury received during either in practice or a game, the Board of Education will endeavor to furnish temporary first aid only. The school cannot perform or be held responsible for any medical attention which might be required, nor can it assume or be liable for loss or damage suffered by reason of injuries to pupils. I/we acknowledge that parents are obligated to pay for professional medical and/or related services; the school shall not be liable of payment of such services. If, in case of injury, the parent cannot be located, the staff is authorized to use their best judgment in obtaining competent medical care and service. I/we acknowledge that insurance coverage is recommended for all participants at parent’s expense and insurance information is available in the main office of the school.
(7) Due to facility size, physical contact between participants, and close proximity between participants, sponsors, and coaches, along with other factors, participation middle school athletics and activities in Lincoln Public School facilities may expose students to infectious diseases, such as for example COVID-19 or other communicable health epidemics and conditions, which, if contracted by a student, could result in serious sickness, illness, disability, and even death. Even the most stringent preventative precautions, including disinfecting, social-distancing or otherwise, cannot protect against the possible exposure to infectious diseases and contraction of illnesses with such potentially life-threatening consequences.
(8) Student agrees to attend all practices and games and to communicate with his/her coach if you cannot be at practices or games.
(9) I/we understand good sportsmanship is expected of all middle school athletes and spectators at games and practices. Unsportsmanlike actions at practices and/or games toward teammates, coaches, officials, and persons from opposing teams or schools may result in removal from the activity. I/we understand all school rules, and Lincoln Public School Rights and Responsibilities apply to athletes at practices and games.

Having read the warnings in paragraph (6, 7) above and understanding the potential risk of injury and/or illness, we hereby give permission for Student to practice and compete in middle school activities. We acknowledge that we have read paragraphs (1) through (9) above and agree to the terms thereof.
